About The Position

This role involves delivering exceptional patient care by providing high-quality, direct nursing care rooted in the organization’s proven clinical standards and safety protocols. The RN will act as the central hub for the interdisciplinary team, ensuring alignment among specialists, physicians, and therapists for a well-coordinated patient experience. Responsibilities include assessing, recording, and communicating changes in patient conditions to meet evolving needs in real time, accurately interpreting and executing physician and nursing orders, and collaboratively assessing and coordinating comprehensive discharge plans with the healthcare team to ensure a safe and successful transition home.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Constantly assess, record, and communicate changes in patient conditions, ensuring their evolving needs are met in real time.
Accurately interpret and carry out physician and nursing orders, translating them into actionable, day-to-day recovery steps.
Collaboratively assess and coordinate comprehensive discharge plans alongside the healthcare team, ensuring patients are set up for a safe, successful transition home.
