Job Summary and Responsibilities As our Clinic RN, you will assume responsibility for facilitating, communicating, and collaborating with the healthcare team and patients/families, utilizing best practices to maximize patient engagement and self-management efforts. Every day you will promote the optimal health, well-being, and safety of patients through the nursing process, adhering to patient care standards, guidelines, and the State Nurse Practice Act, all while demonstrating personal accountability for relationship-based care and organizational mission. To be successful in this role, you will be a Registered Nurse with strong clinical nursing acumen in ambulatory care, knowledge of community health concepts, and expertise in case management and care coordination.
