About The Position

Under the direction of the Assistant Director of Nursing, this role is responsible for ensuring the proper delivery of nursing services in compliance with OPWDD and DOH regulation. The position practices and promotes the agency’s mission statement in every interaction. The RN provides hands-on clinical support to people supported, including assessments, medication administration, responding to emergencies, and performing nursing skills within their scope of practice. They also provide clinical direction to LPN & AMAP staff and ensure nursing skills are performed to standards. The role involves ensuring the implementation of primary nursing principles in accordance with OPWDD and DOH regulation, ensuring all health needs are met in the assigned home. The position requires being on call 24/7 on a rotating schedule to assist with medical concerns when an RN is not on-site. Responsibilities include completing a plan of nursing services (PONS) for each person supported, ensuring all required documents are thorough and up-to-date, and initiating/completing training annually or when a person's status changes. The RN ensures all consultations, lab work, medical visits, and medications are completed as ordered for their caseload. They participate in nursing staff meetings, team meetings, and administrative/management meetings to communicate medical information and seek recommendations for improving medical care. The role ensures the nursing department adheres to all ICF regulations and participates in ongoing surveys by OPWDD. The RN confers with individuals receiving supports and services and their family members/correspondents to discuss medical concerns. They also ensure work-related safety procedures are in place and adhered to by staff regarding workplace safety and infection control. This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and is subject to a fingerprint/criminal background check. The RN reports to the Justice Center as a mandated reporter for suspected abuse, neglect, or hazardous work environments and follows agency incident policies. Adherence to all regulations, agency policies, the employee handbook, and training requirements is expected. Other duties as instructed by the supervisor will be performed.

Responsibilities

  • Practices and promotes Citizens mission statement in every interaction.
  • Provides hands on clinical support to people supported including but not limited to assessments, medication administration, responding to emergencies, nursing skills within scope of practice.
  • Provides clinical direction to LPN & AMAP staff and ensure that nursing skills are be performed to standards.
  • Ensures the implementation of proper principles of primary nursing, in accordance with OPWDD and DOH regulation, ensuring that all people’s health needs are met in the home in which they are assigned.
  • Is on call to the program on a rotating 24/7 schedule to assist with triage of medical concerns when an RN isn’t immediately on site.
  • Completes a plan of nursing services (PONS) for each person supported on their caseload and ensures that all required documents are thorough, complete and up to date (i.e. CFA, PONS, Braden scale).
  • Initiate and complete training every time there is a change in a person’s status and no less than annually.
  • Ensures that all consultations, lab work, medical visits, medications etc are completed as ordered for a designated caseload.
  • Participate in all nursing staff meetings & attend team meetings, administrative and management meetings to communicate medical information as needed and seek recommendations to improve medical care to individuals in each program.
  • Ensures that nursing department adherence to all ICF regulations and participates in ongoing surveys by OPWDD.
  • Confers with individuals who receive supports and services and their family members/correspondents by telephone and scheduled treatment team meetings to serve as an insightful resource and to discuss their specific questions related to their medical concerns.
  • Ensure that work-related safety procedures are in place and are being adhered to by staff as it relates to workplace safety and infection control.
  • Reports to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect or a potentially hazardous work environment.
  • Follows all agency incident policy and procedures.
  • Adheres to all regulations, agency policy and procedures, the employee handbook, and agency and program training requirements.
  • Performs other duties as instructed by supervisor.
