About The Position

Advanced Home Health and Hospice of Cleveland is hiring a PRN After Hours On-Call RN Nurse Case Manager to provide skilled, compassionate support to patients and families outside of standard business hours. This role is ideal for an experienced RN who is comfortable working independently, managing patient needs in the home, and supporting quality care with professionalism, urgency, and empathy. This position offers a unique opportunity to combine home health and hospice case management with after-hours responsiveness. We are looking for a nurse who can confidently assess patient needs, provide skilled nursing care, communicate clearly with families and the care team, and help ensure comfort, safety, and continuity of care during evenings, nights, and weekends.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation for home visits.
• Current CPR certification.

Nice To Haves

• At least 1 year of experience preferred.
• Experience with electronic medical record systems and documentation preferred.

Responsibilities

• Provide skilled, compassionate support to patients and families outside of standard business hours.
• Manage patient needs in the home.

• Provide skilled nursing care.

• Ensure comfort, safety, and continuity of care during evenings, nights, and weekends.
• Coordinate care.
• Support comfort and safety across home health and hospice services.
• Maintain accurate, timely documentation.
• Lift and transfer patients and carry equipment or supplies up to 50 pounds.
• Stand, bend, squat, and walk for extended periods during home visits.
• Use fine motor skills for documentation and patient care tasks.
• Respond swiftly and efficiently to urgent or emergency situations.
• Maintain regular attendance and timeliness.
• Work calmly and effectively in high-stress situations.
