About The Position

We are seeking a Motivated RN to join our team in Utah County as a Case Manager to assist with performing 60 day recertification assessments, admission visits, resumption of cares, CNA supervision, routine nursing visits and care coordination. This person will have a clear path of career trajectory and growth within Amazing Care alongside hands-on training. The schedule is flexible! Hourly pay for case management time. Pay per visit when in the field performing visits in the home. Spanish Fork - Lehi areas.

Requirements

• Active RN License.
• Current BLS Certification.
• Valid Driver's License and Vehicle Insurance.
• Strong time management and prioritization skills.
• Excellent communication, assessment skills and attention to detail.

Nice To Haves

• Previous Home Health experience preferred.

Responsibilities

• Administer medication, treatments, and therapies as prescribed by the physician.
• Monitor patient's condition and report any changes to the healthcare team.
• Assist with activities of daily living and provide emotional support to patients and their families.
• Collaborate with other healthcare professionals to ensure optimal patient care.
• Maintain accurate and complete medical records using web-based electronic medical records system.
• Performing 60 day recertification assessments
