About The Position

The RN Case Manager at HOMECARE COMPANY OF TENNESSEE LLC is responsible for coordinating and managing patient care in a homecare setting. Requirements

  • Active and unrestricted Registered Nurse (RN) license in the state of Tennessee.
  • Minimum of 2 years of clinical nursing experience, preferably in a home care or case management setting.
  • Proven experience in developing care plans and coordinating patient care across multi-disciplinary teams.
  • Experience with electronic health records (EHR) and case management software.
  • Strong assessment and critical thinking skills to evaluate patient needs and develop effective care plans.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare professionals.
  • Ability to work independently and manage a caseload efficiently while maintaining high standards of care.
  • Proficient in time management and organizational skills to prioritize tasks and meet deadlines.
  • Knowledge of healthcare regulations and compliance standards related to home care services.
  • Compassionate and empathetic approach to patient care, with a focus on improving patient outcomes and quality of life.

  • Preferred Bachelor of Science in Nursing (BSN) from an accredited institution.

Responsibilities

  • Conduct comprehensive assessments of patients to determine their healthcare needs and develop individualized care plans.
  • Coordinate and manage patient care by collaborating with healthcare providers, patients, and families to ensure continuity of care.
  • Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
  • Educate patients and their families on healthcare needs, treatment plans, and available resources to promote self-management and independence.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory standards and company policies.
  • Advocate for patients by identifying and addressing any barriers to care and facilitating access to necessary healthcare services.
  • Participate in interdisciplinary team meetings to discuss patient care plans and collaborate on best practices.
  • Provide leadership and guidance to other healthcare staff to ensure the delivery of high-quality patient care.
  • Stay informed about the latest developments in home healthcare and integrate evidence-based practices into patient care.
