About The Position

The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services. The RN Care Manager will work with the individual patient, their families, providers and interdisciplinary care team to develop and implement individual care plans and provide education and self-management support to patients' for their optimal functioning and to improve collaborative coordination of care to reduce waste and inefficiency within the concept of the patient centered medical home. Conducts transitional care outreach to patients within 48 hours of a hospital discharge and assists with effective coordination of services internally and externally. The RN Care Manager achieves targeted outcomes guided by the organization's strategic initiatives, value-based program measures/activities, and as determined within the service line or department.

Requirements

  • Baccalaureate Degree, ADN, or Diploma RN Degree
  • 3 years RN experience with current BLS certification.
  • Valid Arkansas Registered Nursing License

Nice To Haves

  • Certification in Case Management (e.g. CCM, ANCC) or obtain within 1 year of hire date.
  • BSN preferred
  • Care Management experience in an Ambulatory setting

Responsibilities

  • Initiates referrals to internal and external interdisciplinary teams by utilizing pre-established, evidence-based screening criteria to coordinate patient admission, assignment to designated coverage area, target length of stay, and coordinates the discharge planning process in collaboration with the interdisciplinary team.
  • Documents communication and concerns about coordination of care and services.
  • Communicates level of care or admission status when appropriate criteria are not met for inpatient, observation, or continued stay review; and escalates appropriate cases to the Physician Advisor for secondary review process.
  • Participates in the Performance Improvement process by evaluating the quality and effectiveness of nursing practice using current relevant data, evidence of best practices, and compliance measures.
  • Implements Point of Care (POC) employing an interdisciplinary approach to Patient and Family Centered Care by using medical records to collaborate and communicate with the multidisciplinary team.
  • Reports variances in care, systems, and processes through daily communications with the Floor Care Manager of the assigned area.
  • Serves as a resource for professional development to other UAMS employees.
  • Participates in the recruitment and retention of staff.
  • Facilitates the development and implementation of evidence-based nursing practice standards in compliance with UAMS expectations.
  • Performs other duties as requested.
