About The Position

The RN Case Manager is responsible for initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs. This includes assessing for risks such as grief, and considering cultural and spiritual needs, as well as verbal and non-verbal communication. The role involves implementing and recommending revisions to the individualized plan of care, collaborating with patients, caregivers, physicians, and healthcare professionals. The RN Case Manager will attend Inter-Disciplinary Group (IDG) meetings, participate in patient care planning, and prioritize needs to promote coordinated care. On-call responsibilities are also a part of this role, requiring timely submission of all documentation and ensuring compliance with all relevant laws and regulations.

Requirements

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance

Nice To Haves

• Two years of experience as an RN preferred in a healthcare/clinical setting

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
