About The Position

Missouri Baptist Sullivan Hospital is seeking a compassionate, organized, and collaborative RN Case Manager to join our team. The RN Case Manager plays a vital role in coordinating patient care, facilitating safe and timely discharges, reducing avoidable admissions, and connecting patients with appropriate post-acute resources. The Social Services Department enhances the delivery of comprehensive services to our patients. Staff provide information on discharge planning options such as home care, assisted living facilities, or nursing home placement. They can discuss financial concerns and resources for assistance.

Requirements

• RN
• Nursing Diploma/Associate's - Nursing
• <2 years experience
• No Supervisor Experience

Nice To Haves

• 2 years of clinical RN experience and/or previous case management experience
• BSN
• Ability to work collaboratively in a fast-paced environment
• Prior experience with EPIC
• Bachelor's Degree - Nursing
• 2-5 years experience

Responsibilities

• Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes.
• Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues.
• Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
