The RN/Case Manager acts as a client advocate/case manager to Outpatient Behavioral Health and Vitality for Life clients. This role coordinates referrals to services and resources for clients and manages the care of clients to ensure achievement of quality client outcomes. This position works closely with clinical staff to provide case management, discharge planning, referrals, and care coordination. This position also assists in the identification of appropriate utilization of resources across the continuum of care. This position participates in quality improvement and evaluation processes related to the management of client care. This position collaborates with doctors and other medical professionals to give clients comprehensive care to address client’s physical and psychosocial care needs.
