About The Position
We’re seeking a dedicated Registered Nurse (RN) Case Manager for a 13-week contract assignment in Everett, Washington. This is a great opportunity to work in a dynamic hospital environment with a Level II Trauma Center that has 571 staffed beds across two campuses. The role involves providing care coordination and discharge planning for patients from neonates to geriatrics.
Requirements
- Active RN License (pending accepted)
- BLS Certification
- Minimum 1 year of experience
- Acute Care experience
- Discharge Planning experience
- Care Coordination experience
Nice To Haves
- Med-Surg experience
- LLOS experience
- ED Case Management experience
- ICU exposure
- Emergency Department exposure
Responsibilities
- Care coordination and discharge planning
- Conduct needs assessments and arrange DME
- Develop and manage patient care plans
- Ensure compliance with CMS guidelines
- Collaborate with interdisciplinary teams
- Float across all hospital units and both campuses
