About The Position
Baywood Home Care is looking for RN Case Managers to deliver personalized care across the Twin Cities metro area, with a company vehicle and benefits provided. This is a full-time position, Monday - Friday, with an on-call rotation required. The role involves evaluating client conditions, creating personalized care strategies, collaborating with healthcare providers, revising care approaches, guiding clients and families, and documenting interactions.
Requirements
- Active Registered Nurse (RN) license in the state of MN required
- Home care experience required
- Must be 21 years of age
Responsibilities
- Evaluate each client’s condition and create personalized care strategies to promote overall well-being
- Collaborate with healthcare providers and support staff to deliver and oversee care services
- Continuously revise care approaches based on changes in client status and response to treatment
- Guide clients and families on medical conditions, medications, and effective care routines
- Document all interactions thoroughly while adhering to clinical standards and home care requirements
