About The Position
As an RN Case Manager of Luminos Hospice, you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care. This role is a Full-Time position.
Requirements
- Current State license as a Registered Nurse.
- Current driver’s license.
- Current CPR Certification.
- Two (2) years' experience as a Registered Nurse in a clinical care setting.
- Nursing skills as defined as generally accepted standards of practice.
- Knowledge of state and federal regulations.
- Ability to work independently and make sound decisions.
- Strong attention to detail and organizational habits.
- Excellent verbal and written communication skills.
- Good interpersonal skills.
- Reliable transportation and valid auto liability insurance.
Nice To Haves
- one (1) year full-time hospice preferred.
Responsibilities
- Coordinates and oversees all aspects of patients' home care.
- Collaborates with patient care teams to develop and implement care plans.
- Educates families on patient condition, needs and care.
- Performs head to toe physical assessments.
- Checks vital signs and manages medications.
- Administers and maintains catheters.
- Complete documentation timely and accurately.
- Supervises HHAs.
- Regularly communicates patient progress to the clinical manager, physician, and care team.
- Other duties as assigned.
