About The Position

Hinds LifeCare, a well-respected nonprofit agency specializing in hospice and palliative care, is seeking a Full Time RN Case Manager. This role involves providing skilled palliative and end-of-life care in the community. The RN Case Manager will be part of the Interdisciplinary Team, responsible for developing collaborative care plans and coordinating medical management of hospice services. The position emphasizes promoting the philosophy of hospice by focusing on quality of life and connection with loved ones during a patient’s final days.

Requirements

  • Completed a professional nursing program accredited by the National League for Nursing.
  • One year experience in acute care facility or community health agency.
  • Current RN Licensure and CPR Certification.
  • Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.).
  • Availability to work any of the following schedules: Monday - Friday, Tuesday - Saturday, or Sunday - Thursday. (8am - 4:30pm)

Nice To Haves

  • Prefer completion of a baccalaureate program in nursing.
  • Hospice experience a plus.

Responsibilities
