RN Case Manager

As an RN Case Manager, you will combine clinical excellence with genuine compassion to provide care that honors life and brings comfort. You will deliver personalized nursing care in the patient’s home or care setting, collaborating with the interdisciplinary team to build and follow care plans that reflect patient goals and meet high standards. You will also educate and equip families to help them understand what to expect, how to provide comfort, and how to feel confident in their role during a sacred season of life.
Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Excellent interpersonal skills.
- Must read, write and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.
Nice To Haves
- Hospice experience preferred.
Responsibilities
- Completing thorough assessments to guide care based on each patient’s physical, emotional, and clinical needs.
- Delivering skilled hospice care to help manage pain and improve quality of life.
- Collaborating with the full care team, the patient, and their loved ones to create and follow a unified care plan.
- Documenting care and updates accurately in line with all standards and requirements.
- Educating and encouraging caregivers, offering emotional support, and helping families feel prepared and confident.
