About The Position
Are you an experienced Registered Nurse looking to transition from bedside care into a role focused on care coordination, member advocacy, and improving health outcomes? This fully remote opportunity allows you to support high-risk populations while maintaining a strong work-life balance.
Requirements
- Active, unrestricted Texas RN license in good standing
- Must reside in Texas
- 3+ years of experience in community health, public health, chronic disease management, community nursing, or case management
- 1+ year of experience in care coordination and disease management/patient education
- 3+ years coordinating care with primary care providers
- Outpatient case management experience within a health insurance, payer, or managed care environment
Nice To Haves
- Medicaid Managed Care experience (STAR, CHIP)
- Marketplace or Commercial Health Plan experience
- Knowledge of NCQA, HEDIS, HHSC, and TDI standards
- CCM or other healthcare certifications
- Experience with Epic or other EHR systems
Responsibilities
- Conduct telephonic assessments for high-risk members
- Develop individualized care plans and measurable health goals
- Coordinate services between providers, behavioral health teams, community resources, and members
- Educate members on chronic disease management, preventive care, medications, nutrition, and healthy lifestyle choices
- Support transitions of care and remove barriers to treatment
- Maintain accurate documentation and ensure regulatory compliance
