About The Position

The hospice Long Term Care Registered Nurse (RN) is an integral part of the interdisciplinary care team. The RN delivers care according to accepted standards of practice and the agency’s mission, while addressing patients’ and families’ needs and concerns. The role blends agency services into an integrated program of support and guides the assigned healthcare team by monitoring its professionalism and quality of care.

Requirements

• Licensed Registered Nurse (RN) in the State of Texas.
• BSN or equivalent RN experience.
• Training in Long Term Care and in disease processes leading to death, pain control, and management of other symptoms.
• Strong EMR and documentation skills, with compliance awareness.
• Highly organized and detail oriented.
• Effective written and verbal communication skills.
• Ability to travel using reliable transportation.
• Clean background and drug screen.

Nice To Haves

• Experience serving patients in Long Term Care settings, including nursing facilities, assisted living facilities, memory care, and personal care homes.
