About The Position

BrightStar Care is looking for a dedicated and compassionate RN Case Manager to provide in-home infusion and injection services to patients in the Duluth area. This flexible position requires availability to dedicate a few days a week to seeing clients and traveling within a 30-mile radius of Duluth. Mileage is reimbursed. Requirements

Active RN license in the state of Minnesota.
Exceptional interpersonal and communication skills.

Nice To Haves

Previous experience in a home health or acute care setting is advantageous.

Responsibilities

Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.
