About The Position
This role is for a Floating RN Case Manager who can step into different territories and help teams stay on track. The nurse will stabilize caseloads, support transitions, and ensure patients receive consistent, high-quality care. This position offers the opportunity to gain exposure to multiple teams, patient populations, and care environments, with a variety of tasks each week. The role combines the benefits of home health, such as 1:1 care and autonomy, with the challenge of being a go-to nurse for regional coverage.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Strong clinical judgment and ability to adapt quickly in new environments
- Confident communicator and problem-solver
- Reliable transportation and willingness to travel
Nice To Haves
- Experience in home health preferred, but not required
Responsibilities
- Step into territories needing support due to census changes or coverage gaps
- Provide skilled nursing care, assessments, and care planning
- Complete OASIS visits and coordinate interdisciplinary care
- Help maintain continuity and quality across teams
- Educate patients and families while building trust quickly
- Travel throughout the metro with flexibility and independence
