RN Case Manager - Home Health/Hospice

St. Luke’s Health System in McCall, ID is seeking a Home Health Case Manager to join our Home Health team. The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area. As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.
Requirements
- ASN or BSN in Nursing
- 2 years of RN experience
- Current RN license in the state of Idaho
- Basic Life Support (BLS) certification
- Valid driver’s license and proof of auto insurance
Nice To Haves
- Home Health or Acute Care experience preferred
Responsibilities
