About The Position

The RN Case Manager provides direct skilled nursing care to patients in accordance with physician's orders. This role involves assessing patient response to care, educating patients and families, and collaborating with a multidisciplinary team. The position requires maintaining visit productivity and documenting all services rendered, changes in patient condition, and communications with healthcare providers. Requirements

• Current, valid RN license issued by NYS Department of Education.
• Valid and unexpired NYS Driver’s License.
• Current CPR certification.
• Strong communication and documentation skills.
• Critical thinking skills, and ability to function autonomously.
• Clinical Competency of nursing skills.
• Works as team player and collaborates with the patient care team.

Nice To Haves

• Community health experience preferred.

Responsibilities

• Administer direct skilled nursing care to patients, in accordance with physician’s orders.
• Provide hands-on, skilled nursing care in accordance with physician’s orders.
• Provide health education to the patient and family.
• Assess patient response to the plan of care interventions and communicate changes to the case manager.
• Collaborate and share findings with the multidisciplinary care team.
• Document services rendered to the patient, note changes in the patient’s condition and/or family and home situations.
• Document communication with case manager and physician.
• Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager.
• Other duties as assigned.
