About The Position

The RN Care Ally - Nurse Navigator is a new role unique to Duly Health and Care, which will transform how health and care is delivered. This nursing position will be a member of the care team that will be focused on providing personalized one-on-one patient care. As the first point of contact for the patient, the Care Ally RN will help the patient navigate their clinic visit, coordinate their care to reduce gaps in understanding, and ensure high quality patient outcomes. As a dedicated care navigator, the Care Ally RN demonstrates proactive and ongoing guidance, provides a bridge of understanding between the clinical and patient worlds, and in so doing helps patients set, keep, and achieve their health goals. By fostering a cohesive experience and facilitating access for patients, the Care Ally RN will be connected to, if not housed, within the clinic to which their patients' PCP belongs.

Requirements

  • Licensed RN
  • Current CPR certification must be maintained (BLS)
  • Associates required in Nursing
  • 3-5 years’ experience in Nursing
  • Experience interacting with, and relating to, people of varying educational levels and backgrounds
  • Experience providing health information clearly and succinctly
  • Adept at listening, responsiveness, empathy, and maintaining confidentiality
  • Ability to flex and rapidly adjust to changing conditions and scenarios
  • Capacity to remain calm under pressure and drive high value solutions
  • Talented at providing and delivering superior patient experiences
  • Able to adjust treatment, instruction, and education to meet the patient's age and cultural background
  • Aptitude to learn and become proficient with EMR software
  • Basic knowledge of Microsoft Office suite
  • Previous work experience in a clinical setting required

Nice To Haves

  • Bachelors Preferred in Nursing
  • Experience with Value Based Care a plus
  • Prior Experience Using Epic Preferred
  • Prior experience with electronic scheduling of patient appointments preferred
  • Bilingual in English and Spanish preferred

Responsibilities

  • Serves as a primary point of contact for patients through a combination of face to face and telephonic communication, building a relatable human bridge between the clinical and patient worlds to enhance understanding and build trust
  • Acts as Liaison for patients, families, and their multidisciplinary clinical team to address the population health needs of our patients by assessing for clinical, emotional, spiritual, psychosocial, and financial barriers to care; Once identified, works with care team to find a plan to overcome barriers for successful health outcomes
  • Collaborate directly with primary physician and care team to help educate/reinforce specific disease processes, treatments, and recommendations to mitigate, treat, and properly diagnose based on multidisciplinary care plan.
  • Triages, coordinates, and orders appropriate patient testing under primary care protocols and guidance with heavy focus on managing needs within organization and preventing unnecessary ER visits, hospital admissions or readmissions.
  • Facilitates resolution to patient care gaps and follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients.
  • Participates in relevant quality workgroups to assist in the development of patient education materials, clinical pathways, and workflows to improve patient and provider compliance with meeting health measures
  • Responsible for managing routine correspondence and medical records to referring agencies and primary care providers in accordance with policy
  • Participate in clinical operations with patients, rooming patients, taking vitals, supporting patient needs as assigned
