About The Position

The RN Case Manager is responsible for providing medical care to a manageable caseload of hospice patients in their home settings. This role involves developing a plan of care alongside an Interdisciplinary Team, in line with patient/family needs and goals. The Case Manager coordinates all aspects of home hospice care, including symptom management, spiritual, psychosocial, financial, and bereavement needs. They work closely with the Interdisciplinary Team to ensure patient/family goals are met.

Requirements

  • Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
  • Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA and other federal, state and local laws
  • Must be able to lift and carry up to fifty (50) pounds with or without the use of assistive devices
  • Understand and comply with Coastal Hospice’s mission, values, vision, policies and Employee Handbook

Nice To Haves

  • BSN from an accredited college preferred
  • CHPN certification expected within two (2) years of employment

Responsibilities

  • Developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
  • Utilizing equipment and technology skillfully and being technically competent in nursing procedures
  • Overseeing and/or participating in safe medication handling and practices
  • Educating patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensuring consent and payer forms are completed
  • Acting as a patient advocate, providing support, information and direct care
  • Triaging phone calls to identify patient/family needs and notifying Admission Services of referrals
  • Overseeing and delegating assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
  • Participating in weekend, holiday, on call and/or evening/night rotations
  • Maintaining appropriate licenses and adhering to continuing education obligations
  • Ability to cover different areas of hospice care to assure organizational needs are met
  • Inpatient Unit RN: Provides medical care to hospice patients in an inpatient setting
  • Completes assessments, administers medications, and manages plan of care for patients during their shift
  • Coordinates discharge planning with family and medical provider
  • Extended Services RN: Provides medical care to patients in their home environment based on triaged needs
  • Utilizes nursing assessment and symptom management skills to assist with urgent after hours needs
  • Educates patient/caregiver on plan of care in coordination with the on-call medical provider
