RN Case Manager
About The Position
Shenandoah Community Health (SCH) is a Federally Qualified Health Center dedicated to improving healthcare access by addressing challenges such as cost, transportation, and language barriers. We offer a wide array of services including primary care, women's health, behavioral health, and oral health, complemented by on-site pharmacy, labs, and radiology. This community-based nursing role is part of the Care Coordination team and focuses on helping patients overcome barriers to care, connect with necessary resources, and enhance their health outcomes. Requirements
- Current nursing license in the state where services are provided; RN preferred.
- BLS/CPR certification required or ability to obtain within required timeframe.
- Strong communication, organization, documentation, and teamwork skills.
- Valid driver’s license, reliable transportation, and ability to travel locally as needed.
Nice To Haves
- Experience in healthcare, care coordination, case management, community outreach, or social services preferred.
- Bilingual English/Spanish skills preferred but not required.
Responsibilities
- Provide patient outreach, education, and care coordination in clinic, community, and patient home settings.
- Assess barriers to care such as transportation, food access, housing, insurance, medications, and health literacy.
- Connect patients with medical, behavioral health, social service, and community resources.
- Collaborate with providers, care teams, social workers, and community partners.
- Document outreach, referrals, care plans, and patient progress accurately and timely.
