About The Position

Logan Health Medical Center is seeking a full-time Seasonal RN to join their Case Management Team. This is a seasonal opportunity for a minimum of a 13-week period, offering a 15% premium and additional night and weekend shift differential pay. The role is based in scenic Northwest Montana, near Glacier National Park, ski resorts, and Flathead Lake. The position involves facilitating care coordination and discharge planning, managing inpatient utilization, and collaborating with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.

Requirements

Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana.
Minimum of two (2) years’ RN experience in a similar position within a medical setting (e.g. physician office, clinic, or hospital).
RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care, whichever comes first.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Full-Time work schedule required for no less than 13-week period.
Seasonal RN must have 2 years applicable nursing experience to the unit assigned.

Nice To Haves

BSN preferred.
Minimum of one (1) year case management experience preferred.

Responsibilities

Facilitate Care Coordination and Discharge Planning, overseeing the transition of care from inpatient to outpatient settings.
Manage and review inpatient services for appropriate utilization of resources and adherence to guidelines (Inpatient Utilization Management).
Actively participate in interdisciplinary team meetings to develop and implement effective discharge plans.
Identify clinical and social needs of patients related to admission and/or discharge and formulate a plan for transition to the least restrictive environment.
Collaborate with stakeholders to address social determinants of health (SDOH) and develop admission and/or discharge plans.
Identify patients at risk for readmission and work with the interdisciplinary team to formulate and implement discharge plans to decrease readmission risk.
Act as a resource to providers, patients, caregivers, and the interdisciplinary team regarding community resources and reimbursement.
Provide for the physical, psychological, and cultural needs appropriate to the age, ethnicity, and special needs of the patient.
Acknowledge and advocate for a patient’s right to self-determination and informed consent.
Reflect ownership and professionalism towards Case Management, identifying areas for improvement and developing solutions.
Respond to and provide direction in a changing workload in a positive and professional manner.
Promote the use of the most appropriate setting and level of care to optimize patient outcomes and ensure efficient use of healthcare resources.
Identify and obtain necessary provider documentation and orders for compliance with all applicable Logan Health, State, and Federal requirements related to Utilization Review, Discharge Planning, and Care Coordination.
Communicate frequently with patient and family/support to promote the development of a patient-focused plan and ensure involvement in care decisions.
Maintain regular and consistent attendance as scheduled.
