About The Position

National Church Residences Home Health division, a Home Health & Hospice agency in Central Ohio, is seeking a RN Case Manager to join their team. This full-time position is Monday-Friday, 8:30 am - 5:00 pm, with occasional on-call weekends and typically two holidays per year. The role involves mileage reimbursement at $.72 per mile. The RN Case Manager will primarily focus on admissions and will share in the vision to advance better living and care for seniors.

Responsibilities

  • Assuring the development, implementation, and updates of the individualized plan of care, including communication with physicians and integration of orders, especially for medications.
  • Developing individualized plans of care with client, caregiver, and client representative involvement, providing education, mentoring, and support, and notifying them of necessary changes.
  • Using health assessment data to determine problems, goals, and interventions, and communicating with community health providers and facility staff to coordinate the care plan.
  • Working with the interdisciplinary team and physician/physician extender to establish, monitor, and document ongoing home health eligibility.
  • Completing an initial assessment of client, caregiver, and client representative to determine home care needs, performing a complete physical assessment, and obtaining history.
  • Initiating appropriate preventive and rehabilitative nursing procedures, and administering medications and treatments as prescribed.
  • Communicating client needs to the physician/physician extender, reporting changes in condition, and obtaining/receiving physician’s/physician extender’s orders.
  • Preparing clinical notes and updating the primary physician/physician extender when necessary.
  • Providing direct client care as defined in the State Nurse Practice Act.
  • Educating and mentoring the client, caregiver, and client representative in providing care-related needs per the plan of care, and providing support and education on end-of-life issues.
  • Regularly re-evaluating client nursing needs and initiating the plan of care, making necessary revisions as client status and needs change.
  • Coordinating discharge planning in conjunction with interdisciplinary members when appropriate.
  • Attending and participating in scheduled Interdisciplinary team meetings to coordinate care plans, follow-up on changes, and problem-solve.
  • Responsible for the instruction, evaluation, plan development, and supervision of the LPN/LVN, HH/Hospice aides per conditions of participation and policy, including competency evaluation.
