About The Position

The RN Case Manager assists with coordination of services across the continuum of care while balancing quality patient care with cost-effective use of resources. The role coordinates admission, treatment, and discharge plans, supports appropriate utilization of hospital services, helps ensure care is provided at the appropriate level, and serves as a liaison to support appropriate reimbursement for hospital stays and services. The position is full-time, 40 hours per week, on days from 8:00 a.m. to 4:30 p.m. The role is with Vanderbilt Health’s VCH Case Management organization in Clarksville. Requirements

Registered Nurse licensure.
Graduate of an approved discipline-specific program.
Three years of relevant work experience.
Intermediate capability in nursing patient assessment and evaluation, evidence-based practice, patient education, case management, and treatment planning.

Responsibilities

Coordinate admission, treatment, and discharge plans and ensure proper utilization of hospital services.
Ensure care is provided at the most appropriate level to meet patient care needs.
Serve as a liaison to ensure appropriate reimbursement for hospital stays and services.
Conduct patient assessments and evaluations, including patient interviews and physical examinations.
Integrate evidence from multiple sources and share knowledge with peers and clinical team members.
Deliver patient and family education.
Coordinate complex clinical care through case management and treatment planning.
Provide informal guidance, support, coaching, and instruction to team members.
