About The Position

The RN Case Manager develops and maintains a therapeutic relationship throughout the healthcare continuum. This role provides individualized care to promote physical and psychosocial comfort, assesses patient and family readiness, and involves them in planning and implementing care. The Case Manager facilitates the patient and family’s right to receive quality, cost-effective care by mobilizing appropriate resources and adapting practice to the latest standards based on evidence-based literature. They also participate in quality/process improvement initiatives, risk reduction, and patient safety efforts, while meeting regulatory and mandatory requirements. Additionally, the RN Case Manager demonstrates a commitment to meeting the learning needs of patients and families, utilizing appropriate resources, and assisting in creating an environment that promotes educational growth for nursing peers and colleagues. The role involves using the nursing process as a problem-solving model, recognizing changes in clinical situations, evaluating and implementing changes to the plan of care, and making sound clinical decisions for patients with complex problems. They prioritize and organize patient care, initiate referrals and discharge planning, and actively coordinate utilizing an interdisciplinary approach. Ethical decision-making, monitoring for confidentiality, assuring appropriate business conduct, and delegating correctly to unlicensed assistive personnel are key aspects of this position. The RN Case Manager also demonstrates respect and understanding for peers and other clinical disciplines, participating as an effective member of the patient care team. They are committed to the development of self and others, participating in unit meetings and activities that improve patient care or administrative systems, and utilizing available resources to maintain a current knowledge base relevant to practice. Effective communication, feedback, and conflict resolution skills are essential for promoting collaboration among healthcare team members. The role requires the ability to critique own nursing practice and achieve required certifications or a Bachelor's Degree, earning a minimum of 10 contact hours per year.

Requirements

  • Nurse, Registered (RN)
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Graduate from an accredited school of nursing
  • Earn 10 continuing education credits annually
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
