About The Position

Parkland Health Center, part of BJC HealthCare, is seeking an RN Case Manager to join their Care Coordination Department. This role is responsible for coordinating, negotiating, procuring, and managing patient care to achieve quality, cost-effective outcomes. The Case Manager will collaborate with physicians and interdisciplinary staff to identify and resolve issues, and will provide consultation regarding Utilization Management and Performance Improvement programs. The goal is to ensure optimal value for the patient and the reimbursement source, with a focus on accurate and timely processing of patient care and high-quality discharge planning within the local community.

Requirements

• Nursing Diploma/Associate's in Nursing
• Less than 2 years of experience
• RN license

Nice To Haves

• Bachelor's Degree in Nursing
• 2-5 years of experience
• RN experience in inpatient case management

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
