About The Position
A leading home health organization in Salem, Oregon is seeking an RN Case Manager to join its growing clinical team! This is a full-time, permanent, direct-hire position with a Monday-Friday, 8:00 AM - 5:00 PM schedule. Mileage reimbursement and relocation assistance are available. The role involves completing patient assessments, developing and revising care plans, administering medications, educating patients and families, and coordinating care with healthcare teams.
Requirements
- Current Oregon Registered Nurse (RN) license required
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required

- Home health experience preferred
Responsibilities
- Complete patient assessments and develop individualized plans of care
- Monitor patient progress and revise care plans as needed
- Administer medications and treatments according to physician orders
- Educate patients and families regarding health conditions and care plans
- Coordinate care with physicians and interdisciplinary healthcare teams
