As a Hospice RN Case Manager, you will provide comprehensive nursing care and case management services to patients in their homes, assisted living communities, and long-term care facilities. The Hospice Registered Nurse (RN) Case Manager is responsible for coordinating and managing the care of hospice patients while collaborating with physicians, interdisciplinary team members, patients, families, and facility staff. The RN Case Manager develops, implements, and oversees individualized plans of care to ensure patients receive exceptional end-of-life care focused on comfort, symptom management, and quality of life.

Requirements

  • Graduate of an accredited school of nursing.
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required.
  • Current Registered Nurse (RN) license in the State of South Carolina or eligibility to practice under a compact license.
  • Minimum of two (2) years of nursing experience in a hospital, long-term care facility, home health, hospice, or related healthcare setting.
  • Knowledge of hospice philosophy, principles, and end-of-life care practices.
  • Understanding of applicable federal, state, and local hospice regulations.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and make sound clinical decisions.
  • Excellent communication and interpersonal skills.
  • Ability to establish positive relationships with patients, families, physicians, facility staff, and team members.
  • Proficient documentation and organizational skills.
  • Ability to read, write, speak, and understand English.
  • Valid driver's license, reliable transportation, and current automobile insurance.

Nice To Haves

  • Hospice experience strongly preferred.
  • Experience with case management and care coordination preferred.
  • CPR certification preferred.

Responsibilities

  • Provide skilled nursing care and case management services to hospice patients.
  • Conduct comprehensive patient assessments and ongoing evaluations.
  • Develop, coordinate, and oversee individualized plans of care in collaboration with the hospice physician and interdisciplinary team.
  • Manage patient symptoms and medications to promote comfort and quality of life.
  • Conduct hospice admissions and start-of-care visits as assigned.
  • Educate patients, families, caregivers, and facility staff regarding disease progression, symptom management, medications, and hospice services.
  • Collaborate with Care Navigators and other hospice team members to ensure seamless patient care.
  • Provide supervision, guidance, and oversight to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Home Health Aides.
  • Complete supervisory visits and documentation in accordance with hospice regulations and company standards.
  • Participate in interdisciplinary group (IDG) meetings and contribute to care planning discussions.
  • Maintain accurate and timely clinical documentation.
  • Promote patient dignity, comfort, and quality of life throughout the hospice journey.
