RN - Case Manager
About The Position
The Case Manager RN is an expert coordinator in managing care and access. This role integrates utilization management, care coordination, and transition planning functions. The Case Manager has the overall accountability for a designated case load and plans effectively in order to meet patients needs, manage the length of stay, and promote efficient utilization of resources. In collaboration with the interdisciplinary team, the case manager supports the physician in facilitating patient care with the objective of enhancing the quality of patient outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers.
Requirements
- NM State or Multistate License
- BLS Requires at Time of Hire
- ADN (Level I Candidates)
- ADN + 5 Years of Case Management experience or BSN (Level II Candidates)
- MSN or BSN + 5 years of Case Management experience (Level III Candidates)
- MSN + 5 years of experience or BSN + 10 years of experience (Level IV Candidates)
Responsibilities
- Provide clinically-based case management to support the delivery of effective and efficient patient care.
- Integrate utilization management, care coordination, and transition planning functions.
- Manage a designated caseload, planning effectively to meet patient needs, manage length of stay, and promote efficient utilization of resources.
- Collaborate with the interdisciplinary team to support the physician in facilitating patient care.
- Enhance the quality of patient outcomes and patient satisfaction.
- Manage the cost of care and provide timely and accurate information to payers.
