About The Position

Full time RN Case Manager in the inpatient setting at Trinity Health Grand Rapids Hospital. This role will provide case management to our patients and families in the acute care setting. Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.

Requirements

• BSN; or ADN, with BSN to be obtained within 4 years of hire
• Current RN license in the State of Michigan.

Nice To Haves

• Certification in Case Management or other related professional certification.
• At least 2 years of experience in Case Management experience in an acute care hospital
• 5+ years of Case Management experience

Responsibilities

• Provide case management to patients and families in the acute care setting.
