About The Position
The Case Manager assists with coordination of services through the continuum of care and in balancing quality patient care with cost-effective utilization of resources. This is a PRN (as needed) position, requiring flexibility to work during the week and on weekends during day shift hours. The role involves covering for staff on PTO, assisting with call-outs, and picking up shifts due to increased patient volume.

- LIC-Registered Nurse - Licensure-Others
- Licensure-Others
- Relevant Work Experience
- 2 years experience
- Graduate of an approved discipline specific program
Responsibilities
- Coordinates admission, treatment, and discharge plans and ensures proper utilization of hospital services.
- Ensure care is provided in the most appropriate level for meeting patient care needs.
- Serves as a liaison to ensure appropriate reimbursement for hospital stays and services.
