RN Case Manager Home Health Part Time
About The Position
Caring Hands Home Health is hiring a part-time Registered Nurse for in-home care supervisory visits. The role offers flexibility: nurses receive a monthly list of 25–40 visits and can schedule them throughout the month. It may suit a compassionate, reliable, experienced nurse seeking supplemental work. The position involves field work and regular weekly travel to client homes throughout Guilford County. The registered nurse plans, organizes, and directs home care services, drawing on community resources to meet the needs of individuals and families in their homes and communities. The role emphasizes nursing and community health education or experience.
Requirements
- Current Registered Nurse license in North Carolina or a multistate license.
- Graduation from an accredited and approved nursing program, as indicated by a school transcript or diploma.
- Ability to read, write, and communicate effectively in English.
- Ability to work independently and manage time effectively.
- Ability to work as part of a team.
- Strong interpersonal skills.
- Knowledge of how to use electronics.
- Own reliable vehicle.
- Flexibility.
- Ability to travel to assigned cases.
Nice To Haves
- Prior home care experience is strongly preferred but not required.
- Prior experience with electronic medical records (EMR) is preferred.
Responsibilities
- Re-evaluate patients’ nursing needs regularly.
- Provide health care instructions to patients as appropriate to the assessment and plan of care.
- Perform supervisory visits every 90 days, or every 60 days for CAP clients, in assigned areas.
- Communicate changes in a client’s condition to the primary nurse.
- Communicate concerns regarding aides to the field supervisor.
- Plan, organize, and direct home care services to meet the needs of individuals and families in their homes and communities.
