About The Position
This is a contract/travel Registered Nurse position specializing in Case Management for Hospice Care. The role is based in Fort Washington, PA, and involves developing, implementing, and modifying care plans for patients. The Case Manager will collaborate with multidisciplinary teams, document interactions, and ensure compliance with regulations. This is a field-only assignment requiring travel to patient homes and facilities, with mileage reimbursed. The schedule is Monday-Friday, 7 AM to 3 PM, with flexibility, for a duration of 4 weeks.
Requirements
- Active Pennsylvania RN license
- Nurse Compact / Multi-state license (required)
- Minimum 3+ years of hospice care experience
- Must have their own TABLET (smart phone does not work with their software)
- Candidates must have reliable transportation and be willing to travel to/from patient homes and facilities
- Strong communication, critical thinking, and organizational skills
- Case Manager assignments (field only, mileage reimbursed)
Nice To Haves
- Proven clinical experience in care coordination or case management (preferred)
