About The Position

Aegis Healthcare is expanding its Home Health and Hospice Nursing Teams! We are seeking a dedicated RN Case Manager to provide exceptional care to patients in their homes. This role involves conducting comprehensive assessments, collaborating on individualized care plans, managing care delivery, and serving as a resource for families. Aegis Healthcare is Arizona's premier residential healthcare provider, offering a full spectrum of services and committed to enriching every life encountered.

Requirements

• Active Registered Nurse (RN) license in Arizona (or state of practice).
• Effective verbal, written, and time management skills.
• Valid driver's license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with a full range of movement.
• Current CPR certification and negative TB test (we can assist).
• Valid AZ Fingerprint Clearance Card (we can help obtain it).
• All Aegis Healthcare employees must be able to pass a criminal background check.

Nice To Haves

• Previous Home Health or Hospice experience (preferred).

• Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Managing care delivery and maintaining meticulous patient records.
• Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.
