About The Position

This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health is a Magnet Designated Hospital! This role will provide case management to our patients and families in the acute care setting. Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.

Requirements

  • Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire
  • Current RN license in the State of Michigan.
  • At least 2 years of experience in Case Management experience in an acute care hospital

Nice To Haves

  • Certification in Case Management or other related professional certification.
  • 5+ years of Case Management experience

Responsibilities

  • Provide a competent patient assessment by obtaining and interpreting data, making appropriate decisions based upon knowledge and experience and, if applicable, adapting plans accordingly. Utilizes critical thinking in all interactions to identify variances/problems and apply sound decision-making skills. The patient's individual needs and goals are incorporated throughout the case management process.
  • Assess patient needs and plan for discharge by utilizing evidence-based practice, knowledge, and skilled intervention to effectively prioritize and complete comprehensive assessments and facilitate appropriate care coordination through the healthcare continuum.
  • Manage all cases per established organizational standards. Facilitates care coordination and ensures continuity of services to meet patients, family, and staff needs across the healthcare continuum.
  • Monitor and modify a plan of care through interdisciplinary and collaborative processes in conjunction with the health care team members, patient, and family/support systems.
  • Engage in active listening and empathetic communication skills with patients, families, and staff.
  • Coordinate and collaborates with patients, families, multi-disciplinary staff, and the community in care planning, delivery and discharge.
  • Assess for appropriate resource utilization
  • Establish and promotes collaborative relationships with physicians and other members of the health care team. Develops and promotes collegial relationships with external customers.
  • Serve as a patient/family advocate, actively participate in family meetings and team conferences concerning patient care. Supportively allow family members to express ideas concerning care and guide them to appropriate decision making on the patient's behalf.
  • Serve as an educator to patients and families.
