About The Position

Our services include palliative care, expert hospice care, and grief support for individuals and families navigating some of life’s most meaningful moments. At Pathways, our RN Case Managers don’t just provide care—they offer comfort, guidance, and presence during some of life’s most important transitions. As an RN Case Manager, you will lead and coordinate care throughout the hospice journey, build meaningful connections with patients and families, collaborate with physicians, NPs, CNAs, social workers, and clinical leaders, provide holistic care—addressing physical, emotional, and spiritual needs, and work independently in the field while backed by a highly supportive team.

Requirements

  • Graduate of an accredited school of nursing (BSN preferred)
  • 1–2 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
  • Current Colorado RN license and CPR certification
  • Valid Colorado driver’s license, auto insurance, and ability to travel daily
  • Ability to lift up to 35 lbs

Responsibilities

  • Lead and coordinate care throughout the hospice journey

  • Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders
  • Provide holistic care—addressing physical, emotional, and spiritual needs
