RN Case Manager – Cross -Territory Float Role, 1:1 Patient Care
About The Position
This role is built for nurses who like to stay active, pick up additional visits, and be rewarded for their effort. As a Floating RN Case Manager, you step in where demand is highest, which means more opportunities for incentive visits and increased earnings. This role offers higher earning potential through incentive visit pay, placing you where volume and need are highest, resulting in less downtime and more opportunity to maximize your day, while still maintaining 1:1 patient care and autonomy. We operate alongside Interim Healthcare, Lifespark, Adara, Aveanna, and Gentiva, but this role is structured for nurses who want both flexibility and earning upside.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Strong clinical judgment and ability to move efficiently through your day
- Comfortable in a fast-paced, field-based role
- Reliable transportation and willingness to travel
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Float between territories to support high-demand caseloads
- Provide skilled nursing visits, assessments, and care planning
- Complete OASIS visits and coordinate patient care
- Support teams during census growth and coverage gaps
- Track and complete additional visits for incentive earnings
