About The Position

We are seeking a qualified Registered Nurse Case Manager to support Army Special Operations Forces (5th Special Forces Group) under the Preservation of the Force and Family (POTFF) program. This role provides comprehensive, patient-centered case management services to Department of Defense (DoD) beneficiaries, with a focus on coordinated, cost-effective, and evidence-based care that supports the readiness and resilience of military personnel and their families.

Requirements

  • Current, full, active, and unrestricted Registered Nurse (RN) license in any U.S. state or territory
  • Minimum of two (2) years of nurse case management experience
  • Case Manager certification (CCM, ACMA, or ANCC), or ability to obtain within one (1) year of hire
  • Current Basic Life Support (BLS/BCLS) certification through the American Heart Association
  • Ability to obtain and maintain a SECRET security clearance
  • Strong understanding of medical diagnoses and ability to support patients in optimizing function and self-care
  • Ability to manage complex medical, psychosocial, and financial cases
  • Strong communication skills to collaborate with providers, stakeholders, and multidisciplinary teams
  • Knowledge of federal healthcare regulations, professional ethics, and practice standards
  • Familiarity with VA/DoD Clinical Practice Guidelines and utilization review processes
  • Understanding of TRICARE, Medicare, and third-party payer systems
  • Knowledge of healthcare financial systems and resource management
  • Experience using healthcare information systems, data tracking, and reporting tools
  • Knowledge of pharmaceuticals, including therapeutic effects and potential complications
  • Ability to access and coordinate community resources and patient benefit programs

Nice To Haves

  • Experience working with military or Special Operations populations
  • Familiarity with the Military Health System (MHS) and POTFF program
  • Experience supporting discharge planning and care transitions

Responsibilities

  • Conduct and document comprehensive health assessments, including detailed medical histories
  • Develop time-sensitive, individualized care plans based on clinical findings
  • Perform telephonic patient assessments using established protocols to ensure appropriate and cost-effective care
  • Monitor patient progress toward care goals and adjust treatment plans as needed
  • Establish care priorities and evaluate outcomes to ensure coordinated and efficient care delivery
  • Oversee discharge planning to ensure proper timing, sequencing, and continuity of care
  • Coordinate referrals to appropriate healthcare providers, facilities, and community resources
  • Collaborate with healthcare teams, vendors, and stakeholders to support patient care delivery
  • Negotiate and administer service agreements to support treatment goals and compliance
  • Maintain required credentialing and clinical privileges at the assigned Military Treatment Facility (MTF)
