RN Case Manager
About The Position
A leading home health organization in San Marcos, California is seeking an RN Case Manager to join its growing clinical team! This is a full-time, permanent, direct-hire position with a Monday-Friday, 8:00 AM - 5:00 PM schedule. Weekend and after-hours on-call rotations are part of the role. A $12,000 sign-on bonus is available. The organization serves patients throughout Carmel Valley, San Marcos, Carlsbad, and surrounding communities, offering proximity to beautiful Southern California beaches and outdoor recreation, an excellent year-round climate, and family-friendly communities with top-rated schools, shopping, and dining.
Requirements
- Current California RN license required
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required

- Home health experience preferred
Responsibilities
- Complete patient assessments and determine home care needs
- Develop and revise individualized plans of care
- Administer medications and treatments as ordered by physicians
- Educate patients and families regarding treatment plans and healthcare needs
- Coordinate care with physicians and interdisciplinary healthcare teams
