Part Time RN Case Manager- In-Home Infusions/Injections
About The Position
BrightStar Care is looking for a dedicated and compassionate Part Time RN Case Manager to provide in-home infusion and injection services to patients in the Minneapolis and surrounding areas. This flexible position requires availability 3 days a week between 8:00 AM – 5:00 PM, with travel within a 30-mile radius. The ideal candidate would be willing to case manage 4-5 clients. Infusion and injection visits are scheduled based on treatment plans and your availability. Compensation is $70 for 1-hour visits, $50 for the first two hours of longer visits, and $45 per hour thereafter. Mileage pay may be offered on a case-by-case basis. Requirements
- Active RN license in the state of Minnesota.
- Exceptional interpersonal and communication skills.

Nice To Haves
- Previous experience in a home health or acute care setting is advantageous.
Responsibilities
- Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
- Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with all regulatory guidelines.
