About The Position

Continuum Hospice is seeking a Full-Time Hospice RN Case Managers (RNCM) to join their dedicated nursing team providing exceptional end-of-life care throughout the community. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection. Requirements

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR One (1) year experience as a professional nurse within the last three (3) years.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Nice To Haves

• Bilingual in English and Spanish perferred, not required
• Hospice/home health experience preferred but not required.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
• Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
