About The Position

TRU Community Care, founded as Boulder Hospice in 1976, is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. Its mission is to affirm life at every step of the journey with illness and loss, providing a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. This PRN role involves the PACE Clinic RN providing skilled nursing care to PACE program participants. The RN will collaborate with other clinic staff in delivering medical and nursing care, ensuring the highest quality of care in accordance with PACE policy and applicable regulations. The position heavily involves case management and care coordination, alongside regular hands-on patient interaction.

Requirements

• Must possess a current RN license in the state of Colorado
• Must successfully perform position-specific competencies prior to assuming participant care as an RN
• Must successfully pass agency required background checks and drug screening

Nice To Haves

• 2+ years working as an RN, highly preferred
• Experience working with the frail or elderly population, preferred

Responsibilities

• Provide skilled nursing care to PACE program participants
• Work with other clinic staff in the provision of medical and nursing care
• Ensure the highest quality of care in accordance with PACE policy and applicable regulations
• Perform case management and care coordination
• Be hands-on with patients regularly
