About The Position

Desert View Hospital is a 25-bed, acute care hospital located in a rural community. The hospital is committed to providing safe and competent nursing care in accordance with facility policies, standards, and philosophy, emphasizing Patient Centered Care and Service Excellence. The RN Case Manager's primary role is to achieve quality healthcare outcomes by establishing safe, individualized discharge plans and delivering proficient, timely utilization management services. This ensures maximum reimbursement for all patient visits through proactive collaboration with patients, families, and the healthcare team.

Requirements

Graduate of an accredited school of nursing.
A minimum three years experience in varied clinical settings.
Has a current license to practice as a Registered Nurse in the State of Nevada.
Computer proficiency to include word processing, spreadsheet, and data collection/management computer programs.
Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department.
Knowledge of social and physical factors that affect functional status at discharge.
Knowledge of community resources to meet post discharge clinical and social needs.

Nice To Haves

Two years experience in Utilization Review, Utilization Management or Case Management.
