About The Position

Landmark Hospital has an exciting opportunity for a Case Manager to join our dedicated and professional team. Case Manager RN Summary: Responsible for the overall coordination of the patient's plan of care and the resources utilized for the patient's stay. Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum of care. Provides discharge planning through referrals and multidisciplinary planning and serves as a liaison between patient/family and the healthcare team to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks related to the coordination of care. A certain degree of creativity and latitude is required. Ensures that patients' unique needs are met in the most appropriate, timely, and cost-effective manner.

Requirements

  • Active and unrestricted Registered Nurse license
  • 2 years of Case Management experience
  • Ability to develop and maintain positive working relationships.
  • Strong computer skills

Nice To Haves

  • 5 years of Case Management experience
  • Case Management Certification, including CCM, ACM or NACCM
  • LTAC/Acute Care Hospital experience
