About The Position

Four Seasons is a leading provider of hospice and palliative care, recognized for its excellent patient care and commitment to employees. They are seeking an RN Case Manager to coordinate the nursing plan of care, provide direct nursing care, and supervise LPN and Hospice Aide staff. This role involves managing a caseload of approximately 15 patients with varying acuity, with support provided for larger caseloads. The RN Case Manager will practice within their scope of license, assess patients' physical and mental health, plan and evaluate nursing care, and collaborate with other healthcare providers. They will also implement prescribed treatment regimens, provide patient and family teaching, and supervise other nursing personnel. The position requires adherence to safety and infection control methods and may involve participation in on-call services and holiday coverage.

Requirements

  • Valid NC RN license.
  • Valid Driver’s License and proof of automobile insurance.
  • Two years of clinical experience preferred.

Responsibilities

  • Coordinates the nursing plan of care.
  • Provides direct nursing care.
  • Instructs and supervises LPN and Hospice Aide staff.
  • Manages the nursing care for an assigned caseload of approximately 15 patients.
  • Assesses the patient's physical and mental health, including reactions to illnesses and treatment regimens.
  • Records and reports the results of the nursing assessment.
  • Plans, initiates, delivers, and evaluates appropriate nursing acts.
  • Teaches, assigns, or supervises other personnel in implementing the treatment regimen.
  • Collaborates with other health care providers to determine the appropriate patient and family-centered plan.
  • Implements the treatment and pharmaceutical regimen prescribed by authorized personnel.
  • Provides teaching and counseling about the patient's health.
  • Reports and records the plan for care, nursing care given, and the patient's response.
  • Supervises, teaches, and evaluates those who perform nursing functions and administer nursing programs and services.
  • Provides for the maintenance of safe and effective nursing care.
  • Assists in providing 24-hour on-call services, including coverage of at least one holiday per year.
  • Assesses for the need of other disciplines.
  • Performs other similar duties as assigned.
