About The Position

Beyond Hospice, a service of Episcopal Homes, is seeking an experienced and compassionate Registered Nurse Case Manager to provide meaningful end-of-life care and support to patients and their families. Requirements

Current Minnesota Registered Nurse (RN) license
Graduate of an accredited school of nursing
Strong assessment, communication, critical-thinking, and documentation skills
Valid driver’s license, reliable transportation, and current auto insurance

Nice To Haves

Bachelor’s degree in Nursing (BSN)
Two years of acute care nursing experience
Two years of recent hospice or home health experience

Responsibilities

Manage and coordinate care for an assigned group of hospice patients
Complete nursing assessments and individualized plans of care
Provide skilled nursing care focused on comfort, symptom management, and quality of life
Educate and support patients, families, and caregivers throughout the hospice journey
Coordinate medications, medical equipment, supplies, and community resources
Collaborate with physicians, social workers, chaplains, therapists, and other members of the hospice team
Communicate changes in condition and obtain provider orders as needed
Maintain timely and accurate clinical documentation
Participate in interdisciplinary team meetings and the on-call rotation
