Requirements

• A valid Minnesota Registered Nurse license.
• A heartfelt desire to engage directly with patients in their homes, driven by a commitment to positively impact their lives.

Responsibilities

• Manage and schedule patient visits within the community (East Grand Forks and surrounding areas).
• Provide personalized, one-on-one nursing services in patients' homes.
• Make deep connections with patients to ensure they receive the utmost care and attention.
• Act as a vital part of the interdisciplinary team to enhance collaborative, supportive environment and ensure high care standards.
• Guide and inspire caregivers to adhere to care plans with dedication.
